REMINDERS:

A. Before coming for your medical examination, you must lodge your visa application, either with the
Australian Embassy or via eVisa systems 

B. Applicants are advised to take their meals prior to coming over for their
medical examination since food is not allowed within the premises and all
the initial blood and urine tests do not require fasting.

C. FOR PREGNANT Applicants please bring OB_Gyne consent prior to xray.

D. Females within the reproductive age group are advised not to schedule their medical
examination during their menstrual period. Otherwise, they will be required to come
back one week after the last day of their menstruation.

* If original passport is not available:

- a Health Examinations List (HAP letter) (or equivalent documentation from the Department that shows which health examinations are required) and a copy of your passport photo page that has been certified by the department or an AVAC.

- a Health Examinations List (HAP letter) with a photograph attached together with stamp/seal from the Department.

- Non-migrating applicants who does not have a passport or already given to Embassy:

- 2 Government issued ID’ s

- Bureau of Internal Revenue (BIR) ID (laminated version with photo)

- Voter’s ID (laminated version with photo)

- Driver’s license

- Social Security System (SSS) ID

- Postal ID

- Professional Regulations Commission (PRC) ID

- Unified Multi-Purpose ID

- For children (non-migrating)- NSO Birth Certificate and at least one form of identification with photo

(Birth certificates issued by the Local Civil Registrar (LCR)  in lieu of NSO Birth Certificates, are not acceptable.  For an applicant born in a country other than the Philippines, please bring the birth certificate issued by the government in the country of birth.)

D. Please bring all medical records if available

  • For applicants with history of Pulmonary TB treatment- Old Chest Xray films not less than 6 months prior to medical examination 
  • Medical certifcate regarding a previously treated or currently managed clinically significant illness which should contain the following information:
    • Duration and date of treatment
    • surgical procedure or medical intervention done
    • complete final diagnosis
    • outcome of treatment or treatment plans and prognosis

 Medical Examinations Requirements

A. PERMANENT VISA APPLICANTS

5 - 10 years old Medical History and Physical Examination, urinalysis, Tuberculin Skin Test(TST)/ IGRA
11 - 14 years old Medical History and Physical Examination, urinalysis, chest x-ray (CXR)
15 years old and above Medical History and Physical Examination, urinalysis, CXR, HIV

Permanent visa applicants 2-10 years old are required to undertake either a Tuberculin Skin Test (TST) OR a TB Blood Test (Interferon Gamma Release Assay or IGRA)

Tuberculin Skin Test (TST) requires 2 clinic visits. Second visit is between 48 and 72 hours from TST placement.

To avoid TST from becoming invalidated, please be at the clinic at least an hour before the schedule written on your TST return slip.

 The TB Blood Test (Interferon Gamma Release Assay or IGRA) will only require 1 clinic visit if results are negative.

Chest X-ray will be required for children 10 years old and below if they have a positive TST/IGRA result or have exposure to TB Cases or a history of TB disease

B. TEMPORARY VISA APPLICANTS

Nationwide Health Systems Aux Inc only accepts cash payment, Fees must be paid in Philippine Pesos. (Medical Fees quoted are for the basic examinations plus any subsequent necessary investigations and treatment for tuberculosis (TB). Please note that applicants will still have to pay additional fees for any other additional tests/referrals needed for non-TB related issues). Medical fees are subject to change without prior notice.
